Before you come into our Passport Office at your Appointed Time, make sure you have EVERYTHING with you or you will need to make an appointment for another day:

  • A Completed DS-11: Application for a U.S. Passport. You can print the application from the State Department website found here. (Document needs to be filled out in black ink only. DO NOT sign the DS-11 form!)
  • BOTH Parents MUST appear, with the child for who the passport is for.
    • If a parent is unable to be present, the absent parent MUST fill out a DS-3053, get it notarized and get the ORIGINAL document, along with a copy front and back of their ID, to the parent that will be in attendance.
  • Proof of U.S. Citizenship. (Birth Certificate, Naturalization Certificate, US Passport)
    • If a first time applicant, bring the original Birth Certificate or Naturalization Certificate AND a copy (Originals and copies will be sent with the application and originals will be mailed back to you after the State Department has completed the process)
    • If an adult and you are renewing a 10-year passport, you do not come into our facility. Use a DS-82 Renewal Mail-In Application and mail it in yourself.
    • If an adult or a minor 15 years of age or younger, with a 5 year passport , bring the passport AND a copy of your birth certificate.
  • Proof of identity. (Driver's License, Military ID, State Issued ID)
    • If you are an adult, applying for the first time, make a copy front and back of the ID you will be using.
    • If the application is for a minor, both parents MUST present original and photocopies, front and back, of the ID they will be using for all applications for children. If you have 2 children, you must have 2 sets of copies, one for each application. If you have 3 children, you must have 3 sets of copies, one for each application, etc.
  • Appropriate form of payment. (NOTE: These are two separate fees. They CANNOT be on the same check, money order or credit card. Each MUST be paid separately as outlined below. If paying with a  money order, DO NOT make it out for $145 for adults or $105 for minors. You MUST have TWO money orders; one for $110/$80 and then one for $35.)
    • PASSPORT FEE (The State Department at this time has suspended Expedite Services)
      • Adults: Check or Money Order for $110 per application. (ROUTINE SERVICE ONLY) 
      • Minors: Check or Money Order for $80 per application. (ROUTINE SERVICE ONLY)
    • PROCESSING FEE
      • Adults/Minors: Credit Card or Money Order for $35 per application (We are no longer accepting CASH)
  • Appropriate passport photo. Note: Passport photos can be taken at this location for an additional $10 fee. Please add the photo fee to the Processing Fee portion of your payment.

PLEASE NOTE: The City of Laurel Passport Office does not process the DS-82 Renewal Application, which is a Mail-In Only Application.

Application Process For Individuals 16 Years of Age and Older

The applicant is required to complete the application and provide all necessary documents prior to being assisted by a City Passport Agent.  Failure to provide a completed application and the necessary documents listed below will result in significant processing delays and/or denial of your application:

  • Completed Application for a U.S. Passport (Form DS-11 09-2013) (Do Not Sign);
  • Proof of U.S. Citizenship (see instructions on page 2 / #1 of passport application);
  • Front and Back Copy of Proof of Identity (see instructions on page 2 / #2 of passport application);
  • Recent Color Photograph (see instructions on page 2 / #3 of passport application).

First-Time Application For Individuals Under The Age of 16 Years   (Both parents, or the child’s legal guardian(s) must be present)

The applicant’s parent(s) or legal guardian(s) are required to complete the application and provide all necessary documents prior to being assisted by a City Passport Agent. Failure to provide a completed application and the necessary documents listed below will result in significant processing delays and/or denial of your application:

  • Completed Application for a U.S. Passport (Form DS-11 09-2013) (Do Not Sign);
  • Proof of the child’s U.S. citizenship (birth certificate)  (see “special requirements for children” instructions on page 1 of passport application)
    • If re-applying for a new passport for a child under 16 years, all else applies, AS WELL AS need to provide the old passport and a copy of the birth certificate;
  • Evidence of the child’s relationship to parent(s)/guardian(s) (see “special requirements for children” instructions on page 1 of passport application);
  • Parental/guardian government-issued identification (see “special requirements for children” instructions on page 1 of passport application);
  • Recent Color Photograph (see instructions on page 2 / #3 of passport application).

Passport Process To Re-apply For Individuals Under the Age of 16 Years   

Passports issued to individuals under the age of 16 cannot be renewed.  The applicant’s parent(s) or legal guardian(s) are required to re-apply for a passport as if it was their first time applying.

Re-applying for a passport for individuals under the age of 16 cannot be performed by mail. The process must be performed in-person and must include the child’s current passport and birth certificate, along with a completed application package that includes all items listed above in the First-Time Application section.

Application Fees

There are two (2) separate fees paid by the applicant to obtain a passport.  The first fee is the “application” fee paid to the U.S. Department of State and the second fee is the “processing” fee paid to the City of Laurel.  The “processing” fee is a non-refundable payment made to the City of Laurel.  This payment is retained by the City of Laurel whether or not the passport is issued by the U.S. Department of State.

Payment Methods

  • Personal checks and money orders are accepted for payment by the U.S. State Department
  • Cash, credit and debit cards cannot be used for U.S Department of State application payments
  • Cash and Visa/MasterCard credit and debit cards can be used for the City of Laurel processing payments
  • Money Orders payable to the U.S. Department of State may be purchased with a debit card, but not a credit card, at U.S. Postal Facilities
  • Checks or money orders must draw on U.S. financial institutions based in the United States and be payable in U.S. dollars.
